Experts Link GLP-1 Weight Loss to Bone Density Loss, But Say Habits Change the Outcome

A feature published by Women's Health reports that bone loss is one of the least discussed side effects associated with GLP-1 weight-loss medications, according to the clinicians and researchers it interviewed. The article says the issue is not unique to the drugs, and that any route to weight loss carries the same potential.

GLP-1s are prescription medications used for obesity and type 2 diabetes. Bone mineral density is a measure of how much mineral is packed into bone tissue, and low readings are used to define osteopenia and osteoporosis.

Why weight loss affects bone

Sarah J. Wherry, PhD, an assistant professor at the University of Colorado School of Medicine who has researched GLP-1 effects on bone mineral density, told the outlet that bone is a mechanically sensitive tissue that adapts to the load placed on it. Because body weight is a form of load, she says removing weight removes some of the stress on the skeleton.

Registered dietitian-nutritionist Lauren Manaker adds that because the drugs cause people to eat less, they may also take in less calcium, vitamin D and protein. Wherry says the loss of estrogen in perimenopause and menopause compounds the picture, describing estrogen as osteoprotective.

Where the experts disagree

Wherry says maintaining bone density during weight loss is challenging and that how much is lost remains up in the air. Dina Peralta-Reich, MD, board certified in pediatrics and obesity medicine, told the outlet that the medication is not what causes the bone loss and that the problem is excessive or rapid loss, which she defines as more than one to two pounds per week.

Wherry says losing about 8 to 10 percent of body weight through lifestyle intervention is expected to cost about 2 percent of bone mass. She suspects more bone may come off with GLP-1s given how much weight is lost and how quickly, but the article presents that as her expectation rather than a finding.

A 2025 review in Bone Research cited by the outlet reported that weight loss across several methods, including caloric restriction, exercise, drugs and bariatric surgery, was linked to bone loss and reduced bone mineral density.

The experts quoted point to weight-bearing exercise, slower weight loss and adequate protein and calcium as the levers they discuss with patients.
